Exposure and Response Prevention (ERP): the treatment of reference for obsessive-compulsive disorder

A rigorous behavioural protocol for deliberately confronting obsessions without performing compulsive rituals, allowing anxiety to subside.

1 The neurobehavioural trap of OCD

Obsessive-compulsive disorder operates as a loop of enslavement: an intrusive, terrifying obsession, such as 'My hands are contaminated' or 'If I do not check the door, something terrible will happen', triggers overwhelming anxiety. To relieve this distress temporarily, the person performs a compulsive ritual such as repeated washing, incessant checking or mental rituals. The ritual brings brief relief but neurologically reinforces the obsession for the future.

2 The principle of Exposure and Response Prevention

ERP breaks this vicious circle. The protocol deliberately exposes the patient to an anxiety-provoking situation, such as touching a door handle or leaving an object imperfectly aligned, while preventing the usual neutralising ritual. Without the compulsion, the brain experiences anxiety that rises, plateaus and ultimately must fall naturally through the neurobiological process of habituation.

3 A methodical process using a graded hierarchy

Patient and therapist jointly construct a graded exposure hierarchy, rated from 0 to 100 on the SUD scale. Initial exposures target moderate anxiety, first in the consulting room with the clinician's supportive presence and then through exercises at home. As successes accumulate, the person confronts more demanding situations with increasing confidence.

4 The place of this approach within care

This approach requires thorough psychological preparation: it is never imposed by surprise, but jointly developed within a warm therapeutic alliance. It offers a remarkable rate of clinical improvement, freeing patients from the daily hours of torment spent performing rituals.

5 Generalising gains across every context of life

Exposure and response prevention exercises are gradually transferred to all real-life settings, including work, transport and family life. By observing that anxiety always subsides on its own without a ritual, patients permanently regain control over their everyday lives.
